
Outreach workers meet with Homeless individuals and assist them with obtaining housing, healthcare, income, and other mainstream benefits.
Our team provides outreach and case management services to hundreds of Cape Codders facing homelessness each year. Many people across the Cape and Islands are living in the woods, on the streets, or in their cars. Many have complex medical conditions and/or a low income, which makes maintaining housing difficult. Housing Assistance provides support and referrals to anyone experiencing homelessness find and maintain housing to live a more independent life.

The Vinfen Homeless Outreach and Engagement Team (HOET) provides transitional, community‑based support to individuals on Cape Cod who are managing mental health conditions and experiencing homelessness. Our multidisciplinary team conducts direct outreach, completes needs assessments, and connects individuals with medical and psychiatric care, substance use treatment, income assistance, and housing navigation. We coordinate referrals to shelters, transitional housing, and the Department of Mental Health, and offer guidance for long‑term stability planning. By strengthening individualized community connections and reducing barriers to care, HOET works with partners to help individuals move toward sustainable health and housing.

Saint Joseph House
The St. Joseph House is an emergency overnight shelter for men and women located in Hyannis, MA. It has 50 beds and is staffed 24 hours a day.
In addition to the shelter, the program offers a wide range
of supportive services including assessment, job skills training, budgeting information, and access to mental health and other needed services.

Belonging to Each Other supports our Falmouth neighbors experiencing homelessness in their transition to stable housing by providing temporary cold-weather housing and year-round compassionate, supportive services.
We serve individuals over 18 years of age in Falmouth who are experiencing homelessness. We house our residents in rental houses during cold weather months. We can accommodate up to 15 homeless men and women from November through April. As residents transition to more stable housing, others who meet our eligibility criteria will cycle into available spots. Housing case management services are provided year-round to assist people experiencing housing insecurity.

Independence House provides comprehensive support services for individuals affected by domestic and sexual violence in Cape Cod. It features emergency housing resources, counseling, advocacy programs, and a range of educational initiatives aimed at prevention.

Safe Harbor Crisis Center provides shelter, supportive services and advocacy to survivors of domestic abuse and sexual assault, as well as education, awareness and resources to our community.
Help is always available for those impacted by domestic abuse, sexual assault, stalking, or dating violence. The 24-hour support line, 801-444-9161, always has someone at the other end ready to listen and provide help.

Young Adult Services
HPC provides flexible case management and personalized services for youth and young adults under 25 who are unaccompanied and at risk of or experiencing homelessness. Young adults receive one-on-one support
in working towards housing, educational, employment,

Homeless not Hopeless, Inc. (HnH) educates homeless people to live independent, productive lives. We offer a helping hand to those in need of short and long term help. HnH offers supportive housing, educational training and other tools to help our homeless become productive members of society. Our organization is actively managed by men and women who were formerly homeless themselves. They are aware of the problems and issues the homeless face and are equipped to help. HnH’s goal is to help residents move back into their communities as productive members.
