The mission of Duffy Health Center is to provide compassionate and skilled integrated healthcare for persons with complex health and social needs on Cape Cod.

Certified Massachusetts Health Connector Navigators are available at CACCI to assist with applying, enrolling, and shopping for both medical and dental plans. They will help you renew and maintain your Massachusetts Health Connector and MassHealth insurances. They will also help you find primary care providers and other medical and dental services.

Department of Veterans' Services

The Department of Veterans' Services provides support andTown of Falmouth Seal information to veterans and their families throughout Falmouth. We can help you with pensions, filing claims, locating your DD214, filing for Chapter 115 benefits and much more. We are located downstairs in the Falmouth Town Hall. If you have any questions please call us.

Falmouth Senior  Center

The Falmouth Senior Services Department functions as a human services organization for all residents of Falmouth who are at least 60 years old.
The Senior Services Department manages the Senior Center, provides assistance, information on available resources, health services, referrals to other community agencies, education and recreation programs and activities.

There is a special emphasis in promoting Healthy Aging and Enhancing Quality of Life for Seniors.

The Samaritans on Cape Cod and the Islands is a nonprofit organization that provides support to people experiencing crises. Our mission is suicide prevention: we are dedicated to helping people contemplating suicide or personally affected by suicide. Our goal is to be a caring and compassionate listener who, through interaction in a safe and anonymous environment, can rekindle people’s lost sense of hope, de-escalate a situation from a crisis level, and provide the pathway that makes day-to-day living manageable.
There is nothing shameful about feeling suicidal. There may be pain, hopelessness, and feelings of isolation;
but there never needs to be shame. And with help, there can be hope.

ConnectorCare plans offer great coverage with important benefits like doctor visits, prescription medications, and emergency care. ConnectorCare plans have low monthly premiums, low co-pays, and no deductibles. Plus, you may be able to fill certain prescription medicines at no cost for chronic health conditions like asthma, coronary artery disease (a type of heart disease), diabetes, and hypertension (high blood pressure).

There are different ConnectorCare Plan Types, which are based on your income. All of the plans offered for each Plan Type will have the same benefits and co-pays for covered services. You can see the co-pays for different services in the chart on the next page. You can also see examples of the monthly premiums for each Plan Type.
