Find Support at Duffy Health Center
Duffy Health Center is a regional leader in treating alcohol and opioid use disorders in an outpatient setting.
Our providers offer compassionate, knowledgeable care to help you reach your goals on your pathway of recovery.

Duffy Health Center's REACH Mobile Unit brings care, support, and services to members of the community for whom transportation is a challenge. All members of the community can access services on the REACH Mobile Unit, including those who are not Duffy patients. Services are available to individuals in need as well as loved ones looking for support.  Services provided in the unit are free.

Compassionate Recovery Services
Substance use can tear lives and families apart. Community Health Center is here to help with a comprehensive, structured and safe outpatient treatment program.

Community Health Center offers recovery services, including medications for opioid use disorder (MOUD) that serve people who need assistance with recovery from substance use in a compassionate environment.
These services are provided to individuals who receive their primary care at CHC.

ASGCC offers an array of services through our Mobile Outreach Van. For clients in Falmouth and other locations in the Upper Cape, the van provides local access to syringe services, as well as STI/HIV/HCV screenings, safer sex supplies, comprehensive risk assessments, and harm reduction services (including overdose prevention), as well as referrals to medical, mental health, and recovery supports.

Hopeful Outreach of Cape Cod

Our mission is to empower individuals facing substance use disorder and homelessness by offering compassionate support. We offer access to recovery programs and housing resources. We are dedicated to promoting healing, building resilience, and guiding people toward healthier, more fulfilling lives.

​Starting with a boots-on-the-ground outreach approach, we committed ourselves to walking alongside individuals experiencing homelessness and recovery challenges. From the very beginning, our focus has been person-centered meeting people where they are, listening to stories, and offering support with dignity and compassion.

We meet people where they are, offering compassion, resources, and guidance to those experiencing homelessness and individuals facing substance use disorder. Through direct outreach, recovery coaching, and sober living scholarships, we provide practical support that helps people take their next steps toward stability. Our team works within Cape Cod Hospital and Falmouth Hospital as recovery specialists providing access to treatment.

Our team also connects individuals with essential items, financial resources, and personalized case management, ensuring no one has to walk their journey alone. By combining advocacy, community partnerships, and hands-on support, we empower people to rebuild their lives with dignity and hope.
